NORTHWEST JUNIOR JAMBOREE ENTRY FORM
March 10-11, 2012
All entries must be done through the computerized Advanced Event Systems (AES) program which is utulized for all Evergreen Region tournaments, as well as the Volleyball Festival
and national qualifiers (such as PNQ).
If your club does not have an AES account, it must be established prior to attempting tournament entry. Accounts are done on a "club" basis. Log on to www.advancedeventsystem.com and click on
"Create a New Account" for details. We recommend that you create an account as soon as possible,
in order to submit entry requests for the NW Jamboree on the first permissible entry date of
December 10, 2011.
After you have entered your team(s) through the AES program, you must fill out and mail the following form to Team Yakima for each team entered.

Entry Fee is: $275 per team.

Make checks to: Team Yakima (you may submit one check with multiple Team Entry Forms)
Rosters with USAV numbers and uniform numbers can be sent later but must be received by 3/3/2011.

Each team must provide qualified personnel for officiating duties.

All R1’s (up refs) must be adults.

There will be a coaches meeting Friday night prior to the tournament.
